Medicare pays for a dementia care navigator. Almost no family is told.
The program is called GUIDE — Guiding an Improved Dementia Experience. CMS launched it in July 2024 and runs it as an eight-year pilot. Through a participating practice it covers a named dementia care navigator, a 24/7 support line, caregiver training, a written care plan, medication review, referrals to services and respite — at no cost to the family.
Who qualifies: a diagnosis of Alzheimer’s or another dementia · enrolled in Original Medicare Parts A and B · not enrolled in Medicare hospice or PACE · not living in a long-term nursing home.
